Adaptation examination reservation

If you wish to have LASIK, you will need to undergo an adaptation test first.

■ Adaptive test reservation

​ Adaptive test reservation fee 3000 yen
​ *The aptitude test fee will be deducted from the surgery fee after the surgery is confirmed.

◇ Inspection reservation form

  • Desired reservation date and timePlease fill in up to No. 3.​

  • The reservation form accepts reservations up to one week before the desired date of the first consultation. If you are in a hurry, we accept reservations by phone.

  • We will send you an email confirming your reservation.(If you do not receive an email within a few days, please contact us by phone.)

  • In case of cancellation, please contact us by phone 3 days in advance.
